- Application forms are available on the Medical Laboratory website or from the Medical Laboratory Science Program office.
- In addition to the application form, applicants will need to include copies of academic transcripts, a resume detailing volunteer, paid, and unpaid experience, two letters of reference, and a well-written and structured personal statement addressing questions specified in the application form.
- $65 application fee
- Official high school transcript
- Copy of high school diploma or high school equivalency diploma and scores
- Please note that SAT/ACT exams are not required, nor will they be considered through the Spring 2023 admissions cycle.
- For admission purposes, you are considered an international student if you are currently in an F-1 or J-1 non-immigrant status, will need to change your current status to F-1 or J-1, or will need an F-1 or J-1 visa in order to study at CSI.
