The vision of the Health Care Administration and Planning program at Tennessee State University is to become the premier undergraduate program for the academic preparation of health care leaders in the state of Tennessee and the southeast region.

Notarized Affidavit of support and financial statement. These documents must not be older than six (6) months prior to desired semester of enrollment

Official transcripts from colleges/universities attended in the United States

Official transcripts and authorized school records with a course-by-course listing of courses and grades received; such transcripts must have been evaluated by a foreign educational credential agancy, at the student's expense.
