The Public Health and Community Health Education program at the University of Wisconsin La Crosse wants to prepare Public Health and Community Health Educators through the necessary coursework in entry-level public health core areas and health education competencies.

  • $50 Application Fee: Payable with a credit card at the same time you submit an online application.
  • Academic Records: Official transcripts from all secondary and post-secondary institutions attended. Documents issued in a language other than English must be accompanied by a literal English translation.
  • Proof of Funding: Complete an Affidavit of Support with supporting bank statement. Funding must be greater or equal to our estimated costs for year of study.
  • Passport Copy: Submit a copy of your passport page which contains your photo, name, country of origin, birth date, etc.
